Do you provide transportation to medical appointments?
Yes. Transportation to doctor visits, therapy appointments, errands, and social outings is included as part of our comprehensive, ongoing care plans. We provide these services for clients who meet our minimum scheduling requirements (a minimum of 4 hours per visit and 20 hours per week). This ensures consistency, safety, and continuity of care while supporting our clients’ independence in the community.
Can you accommodate special dietary needs?
Yes. We assist with meal planning and preparation for specific diets, including diabetic, low-sodium, renal, and heart-healthy plans.
How do you create a Social Care plan?
Our Care Managers meet with families to understand lifestyle preferences, health needs, and safety concerns. We then design a personalized care plan that supports independence and well-being at home.
Does Medicare cover Social Care services?
No, Medicare does not cover non-medical Social Care services. However, we do work with Long-Term Care Insurance policies and can assist families with documentation and coordination when coverage is available.
